Responsibilities

Communicate with the licensees concerning the operation of the facility
Ensure the provision of services identified in each client’s individual needs and services plan or required by California Code of Regulations, Title 9, Division 1, Chapter 3, Article 3.5, Section 531 through 535 or Title 22, Division 6, Chapter 2
Ensures arrangements for clients to attend available community programs, when clients have needs identified in the needs and services plan which cannot be met by the facility but can be met by community programs. Such arrangements may include but are not limited to, arranging for transportation
Ensure arrangements for special provision of services to clients with disabilities including visual and auditory deficiencies
Ensure the development of an employee work schedule
Ensure provision of staff support to clients in the planning, preparing and service of meals
Ensure provision of staff and/or program support for those clients that are unable to plan, prepare and service meals
Ensures provision of staff support to clients in performing facility maintenance and/or cleaning activities as designated in the client’s treatment/rehabilitation plan
Ensure provision of staff and/or program support for those clients that are unable to perform facility maintenance and/or cleaning activities
Directs provision of program services to CRT clients
Implements a recovery based psychosocial program to optimize patient strengths, skills, and partnerships with family, community, and staff
Assures that client needs are met through program activities
Selects, trains, supervises, evaluates and disciplines assigned staff
Provides ongoing in-service training and continuing education on a regular basis for social services staff and for other clinical staff as needed
Establishes and maintains an on-going relationship with county agencies, health care facilities, placement centers and other resources necessary to affect the continuum of care for clients returning to the community
Represents social services and rehab on various facility committees, including Utilization Review, Peer Review and Quality Assurance
Assist with the discharge planning including: assessing client’s readiness for discharge and coordination planning with treatment team; making appropriate referrals for placement, case management and aftercare services; coordinates benefits (Medi-Cal) with Business Office personnel; and maintains knowledge of community resources

Required

Applicants must have attained the age of 18 years of age
Successfully completing a pre-employment Health Screening and a TB clearance
Receipt of 'clear' or approved Live Scan results and Criminal Background Clearance
Master's degree in social work, Psychology, or Marriage, Family and Child Counseling from an accredited college or university
Graduation from an accredited school of licensed vocational nursing or psychiatric technology required
Two (2) years direct service experience with the mentally ill, preferably adults
Two (2) years supervisory experience in a health care setting
Valid California Driver's License
Demonstrated ability to engage clients and families
Demonstrated understanding of psychiatric illness and its treatment as appropriate for Scope of Practice
Demonstrated understanding of Company's Compliance Program and Code of Ethics
Demonstrated understanding of laws and regulation affecting behavioral health clinical care
Ability to perform clear and concise documentation including use of the EMR where applicable
Ability to drive to and from clients' homes in personal vehicle if applicable
Demonstrated understanding of how to behave in a safe manner while seeing clients in the community or facility as applicable
Demonstrated ability to recognize dangerous behavior by clients and intervene as appropriate
Demonstrated use of Professional Assault Crisis Training (Pro-ACT)
Demonstrated use of Standard Precautions

Preferred

California Registered Intern Clinical Social Worker (LCSW) or Marriage, Family and Child Counselor (LMFT) preferred
Valid California LVN or LPT license
First Aid certification from persons qualified by agencies including but not limited to the American Red Cross
