Senior Administrative Assistant (Case Manager - Homeless initiatives Program)

The City of Fort Lauderdale is seeking experienced, client-centered Case Managers to provide intensive street outreach and comprehensive support to individuals and families experiencing homelessness. This role involves conducting assessments, assisting clients with obtaining necessary services, and collaborating with various community partners to ensure long-term stability for clients.

Responsibilities

Conduct proactive street outreach and provide crisis intervention as needed
Complete client assessments, intakes, and individualized case plans
Assist clients in obtaining identification, benefits, healthcare, and social services
Support clients with housing navigation and long-term stability planning
Collaborate with multidisciplinary partners, including the Fort Lauderdale Police Department Homeless Outreach Team (HOT), Fort Lauderdale Fire Rescue Mobile Integrated Healthcare (MIH) team, and Fort Lauderdale Police Department Co-Responder mental health team
Participate weekly in Community Court and attend meetings/trainings
Maintain accurate documentation and enter data into HMIS and internal tracking systems
Performs related work as required

Required

Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field
Minimum 3 years of case management experience with vulnerable or unsheltered populations
Strong skills in Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
Excellent verbal and written communication skills
Proficiency in Microsoft Excel
Ability to walk long distances and work outdoors in all weather conditions
If claiming Veteran's Preference, candidates must attach a DD214 Member 4 form

Preferred

Masters degree in Social Work, Psychology, Sociology, Human Services, or related field
