Clinical Case Manager

Prince George's County Government provides a dynamic and inclusive workplace dedicated to serving the County's residents. The Clinical Case Manager in the Behavioral Health Services Division will engage with client populations, perform assessments, and develop individualized case plans to support community reentry and health services.

Responsibilities

Links newly released individuals with housing, vocational, and educational opportunities and community services
Performs assessments and screenings, then develops an individualized case plan
Creates connections and referrals to service providers based on needs and makes warm handoffs
Provides ongoing case management
Updates the electronic health record database within 24 hours of each interaction or update
Models useful skills and effective problem-solving strategies
Shares experiences and provides coaching in regard to employment, training, and/or obtaining housing
Supports the client's engagement in mental health and substance abuse treatment services
Provides information on the rights and responsibilities of discharged offenders and satisfying criminal justice system requirements and conditions
Provides practical support to newly released individuals by accompanying them to initial probation meetings, treatment appointments, employment interviews, etc
Makes referrals to community-based service providers
Participates in reentry outreach activities

Qualification

Required

Bachelor's Degree from an accredited college or university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field
Responsible work experience which includes coordination of group activities, preferably including work with disadvantaged individuals
Must possess and maintain a valid Class C driver's license

LCPC – Licensed Clinical Professional Counselor
LCADC – Licensed Clinical Alcohol and Drug Counselor
LGPC – Licensed Graduate Professional
LGADC – Licensed Graduate Alcohol and Drug Counselor
LCSW-C – Licensed Certified Social Worker – Clinical
LCSW – Licensed Certified Social Worker
LMSW – Licensed Master's Social Worker
LCP – Licensed Clinical Psychologist
