Responsibilities

Lead coordinator and supervisor for the Central Office Care Coordination Team
Work hand-in-hand with community provider agencies, community partners, and ODMHSAS program staff to ensure seamless collaboration and support
Coordinate care, offer outreach and engagement services, reduce hospitalizations, crisis stays, ER visits, and enhance overall access to care for individuals
Lead and oversee case staffings, provider meetings, and trainings
Manage data reporting and analysis
Monitor fiscal performance to ensure services are effective, efficient, and impactful

Required

Master's degree and one year of professional experience
Bachelor's degree and two years of professional experience
Equivalent combination of education and experience, substituting one year of professional level experience for each year of the required education

Bachelor's degree and three years of professional experience for Level III
Equivalent combination of education and experience for Level III, substituting one year of professional level experience for each year of the required education

Preferred

Candidates who are fully licensed
Previous supervisory experience
