Case Manager - Youth Services

NEOGOV is seeking a dedicated Case Manager for Youth Services to support youth and adolescents in accessing behavioral health services. The role involves assessing behavioral health needs, coordinating services, and advocating for youth and their families during transitions or crises.

Responsibilities

Assess the behavioral health needs of youth or family members, identify risk factors, and develop and implement a care plan that outlines goals and objectives
Provide support and guidance to youth and families during times of transition or crisis, specifically behavioral health concerns
Coordinate services with schools, hospitals, youth protective services agencies, mental health centers, homeless shelters, local behavioral health providers, and other entities to meet the youth’s needs
Provide youth and family members with a warm handoff to community-based services provided by other professionals such as therapists, psychiatrists, teachers, and any other system providing a service to the client
Serve as an advocate for youth and their associated caregivers and/or family members in accessing and maintaining adequate services
Monitor progress on goals and objectives, communicate with parties involved in the case, and make adjustments as needed
Communicate regularly with youth or their parents about their progress in meeting goals and objectives
Review reports from other professionals involved in the case, such as therapists or teachers, to ensure progress
Use the Unite Us Platform and other referrals to appropriate community resources
Work with youth and family through their care plans and collaboratively work with family to decide on appropriate discharge
Develop and distribute youth-specific behavioral health promotional materials
Establish and maintain regular contact with behavioral health providers and professionals in the community, including other governmental entities
Work cooperatively with other Divisions of the Community Services Department to maximize available resources

Required

High School Diploma or Equivalent
Five (5) years of work experience in social work, community-based navigation, case management or a closely related field
Selected candidate must submit to and pass a County paid pre-employment physical and drug/alcohol screening
Selected candidate must submit to and pass a county paid criminal background screening
Selected candidate must possess and maintain a valid New Mexico Class D Driver's License
