Responsibilities

Ensure consistent implementation of ECM, CS, and CHW service models, workflows, and care coordination practices based on program models, policies, and established workflows
Promote fidelity to program models while supporting individualized, member-centered care
Conduct field-based support, case consultations, and service delivery reviews
Identify variations in practice and implement coaching, practice supports, and workflow improvements to strengthen service consistency and quality
Serve as a secondary level of review for complex care planning and elevate program design, policy interpretation, or payer-related issues to program leadership
Promote trauma-informed, culturally responsive, and equity-focused service delivery practices in daily practice
Lead onboarding and ongoing training to reinforce service standards and expectations set at the program leadership level
Provide structured coaching and reflective supervision to strengthen practice skills
Support the development of staff and emerging supervisors through skill development and practice guidance
Foster a culture of continuous learning, accountability, and professional growth
Provide direct and/or functional supervision, coaching, and practice support to assigned staff
Provide performance feedback and professional development support within assigned supervisory scope
Monitor workload balance, staff readiness, and support needs; coordinate adjustments in consultation with program leadership when structural or staffing model changes are needed
Assist staff in managing high-acuity caseloads and cross-system barriers
Serve as acting program lead in the Program Director’s absence for operational and practice matters, as assigned
Provide real-time guidance and consultation for complex or high-acuity member situations
Support staff in navigating cross-agency coordination involving healthcare, behavioral health, housing, and social services
Assist with crisis response and problem-solving related to service delivery barriers and escalate high-risk or program-impacting matters to program leadership
Support staff in identifying and responding to member safety concerns, mandated reporting situations, and risk-related service issues
Support chart reviews, audits, and quality improvement initiatives
Track service delivery trends and operational performance indicators
Identify compliance risks, documentation concerns, or regulatory questions and escalate to program leadership for review and determination
Promote adherence to professional ethics, boundaries, and standards of practice
Utilize service data and performance trends to inform training, workflow improvements, and practice development
Implement workflows, procedures, and operational processes developed at the program leadership level
Support rollout of new initiatives and service expansions
Identify operational barriers and recommend improvements to program leadership
Collaborate across divisions to support coordinated service delivery in a matrixed environment
Provide practice input to support implementation and optimization of agency care management and documentation systems
Support staff adoption and workflow integration
Relay system improvement needs, staff feedback, and practice challenges to program leadership and project leads
Support coordination with Managed Care Plans, healthcare providers, housing partners, and community organizations at the service-delivery and operational level
Assist with outreach, engagement, and cross-agency collaboration
Provide support during complex cases and route payer-level, contractual, or policy matters to program leadership
Provide operational input and frontline feedback to inform program improvements
Translate program goals into team-level action plans under the direction of program leadership

Required

Bachelor's degree required
Demonstrated experience leading in behavioral health, social services, care coordination, housing support, or community health programs
Experience in ECM, CS, CHW services, case management, or working with high-acuity populations
Demonstrated experience working within complex agency and community service environments
Experience working in regulated health and social service environments with responsibility for maintaining privacy, confidentiality, and data security standards (including HIPAA and related regulations)
Must meet all state-required conditions of employment

Preferred

Master's degree in Social Work, Counseling, Psychology, Public Health, or related field preferred
Knowledge of Medi-Cal, CalAIM, managed care environments, or integrated care models preferred
