Responsibilities
Works with operational Home Health & Hospice leaders (Intake, Scheduling, Quality, Nursing) to identify challenges and opportunities within their areas
Catalog existing reporting from Epic, Qlikview and SHP
Understanding of departments and initiatives leading to improved network integrity
Maintain dashboards for each area to provide insight and support analytics needs
Uses quantitative and qualitative methods to systematically collect and analyze data to collect and analyze data to inform daily operations and long-term strategy
Assist in the creation of a summary template for Home Health and Hospice to collect and present data on referral volume, conversion and quality outcomes
Review trends and opportunities to assist leadership
Submit data summary to internal and external referral sources on routine basis
Provide support to other Integrated Care Management leaders as needed
Creates an understanding of each ICM department to be able to assist with recommendations to existing reporting structures
Qualification
Required
2-5 Years Related Experience Required
Proficient in Excel, Word and PowerPoint
Bachelor's degree required
Preferred
Ideal candidate possesses a combination skilled analytics and knowledge of the healthcare industry
Experience Within Virtua Preferred
Experienced with patient tracking and service utilization programs
