You'll provide support, encouragement, and education to members in complex healthcare situations, such as hospital discharge or admission to a skilled nursing facility, to help them make informed decisions and navigate care options.

As a subject matter expert, you'll provide guidance and support to Care Advocates I and II, or individuals with less experience, assisting with training and trouble-shooting issues, and identifying areas for improvement in program processes and procedures.
