Responsibilities
Conduct telephonic non-clinical assessment(s) to understand member care needs, preferences, and socioeconomic barriers to appropriately triage member needs and complete internal program referrals
Work collaboratively with other Humana associates as a member of Humana’s Care Management Support teams, including case managers, SDOH coordinators, and Community Education Representatives (CER)
Serve as a member of member multi-disciplinary care teams (MDT), as requested
Assist members with scheduling physical health and behavioral health (BH) office visits and addressing barriers to appointment attendance
Perform basic administrative, clerical, and operational support
Transfer inbound member calls to the most appropriate team/department
Support member self-management through the provision of culturally appropriate health education and health coaching.
Periodic travel to our Humana office in Louisville for in-person meetings

Required
Must Reside in Kentucky
2+ Years of Administration support experience in a healthcare industry
Knowledge of Microsoft Word, Excel and Outlook
Exceptional verbal/written communication and interpersonal skills
Ability to use a variety of electronic information applications/software programs
Preferred
Associates Degree
Microsoft Access proficiency
