Responsibilities

Care Coordination: Screen beneficiaries, assess needs, and develop individualized care plans (CCPs).
Advocacy: Ensure access to services, community resources, and support.
Enrollment & Benefits Education: Assist with enrollment and benefits education.
Collaboration: Work with providers to ensure coordinated care.
Compliance: Maintain documentation per HIPAA and accreditation standards.
Reporting: Present complex cases for review with the Medical Director.
Supervision: Oversee non-clinical administrative staff and ensure accurate documentation.

LCSWRNCase ManagementClinical Care ExperienceAutism ServicesCase Management Certification

Required

Licensure as an LCSW or RN in the U.S.
3+ years of direct clinical care experience, preferably in pediatrics, behavioral health, or ASD.
Experience in case management or autism services.
Ability to obtain a case management certification within 4 years.
U.S. Citizenship and eligibility for a DoD background check.
