Responsibilities
Perform data abstraction for assigned facility(ies), ensuring accurate abstraction of cases into required systems (e.g., COMET, TheraDoc, Digital Innovations, NHSN).
Review medical records and abstract information in accordance with regulatory and accreditation standards (e.g., CMS, TJC, NHSN).
Complete abstraction processes in a timely manner to ensure compliance with industry standards.
Conduct edit checks and make necessary adjustments on a timely basis.
Follow standards and guidelines to abstract all reportable cases.
Assist with case follow-up as requested by the team.
Participate in educational activities approved by the Manager and/or Director to maintain clinical knowledge of abstracted measures.
Communicate with the Manager to ensure compliance with data reporting and resolution of errors.
Submit data through the appropriate reporting system within the required timeframes.
Address issues that may result in data rejection and take corrective action to ensure accuracy.
Participate in continuing education programs and annual reorientation sessions.
Adhere to the Code of Conduct, Mission, and Value Statement of the organization.
Other duties as assigned.

Required
2+ years of experience as a Trauma Registrar/Abstractor.
Proficient in ICD-10 data abstractor systems.
Completion of Trauma Registry Course.
Completion of AIS 15 Course.
2+ years of experience with Level 1 or 2 Trauma Centers, with certification in CSTR or CAISS a strong plus.
Experience with EMR/Registries in DI or CDM (trauma).
Familiarity with Meditech Expanse, Epic, or Cerner EMR systems.
In-depth understanding of medical record documentation.
Knowledge of basic medical terminology and physiology.
Ability to navigate medical records to locate relevant documentation.
Ability to extract and abstract data from medical records for reporting purposes.
Proficiency in Microsoft Office applications and basic keyboard skills.
Strong understanding of medical terminology, quality measures, and coding logic.
High school diploma or equivalent.
Preferred
Certification in CSTR or CAISS.
Undergraduate (Associates or Bachelor’s) degree or successful completion of a certified coding program.
At least 1 year of experience in Health Information Management, coding, or health registry abstraction.
