Responsibilities
Lead / support value-based care program development and execution
Lead / support multiple value-based care accounts with physician partners.
Lead / manage cross-functional / departmental teams engaged in identified value-based care initiatives.
Manage & drive value-based care program performance, including key operational and financial metrics
Manage & implement value-based care program initiatives, including risk adjustment, quality metrics, and clinical programs to achieve cost savings
Partner closely with core USRC dialysis operations and clinical teams to achieve value-based care program objectives
Lead / manage meetings; organize pre-meeting agendas, facilitate meetings, maintain efficient/on schedule meetings, post-meeting follow-up, and summary notes.
Report project management updates and timeline progression to VP/SVP, Value Based Care as needed.
Meet Value Based Care project deliverables on time and within scope and budget.
Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.
