Responsibilities
Follow up on referrals for participant-directed services
Support individuals/employers and providers in completing necessary documentation for enrollment
Educate individuals/employers on interacting with Public Partnerships as their fiscal intermediary
Identify potential barriers to enrollment and take necessary steps to resolve
Engage with case management services for timely coordination of service approval
Perform in-home visits of Individuals as applicable by program
Collaborate with stakeholders to minimize enrollment cycle times and ensure timely payments
Assess participants' ability to successfully participate in self-directed services
Provide person-centered program education and guidance to participants
Explain roles and responsibilities for participation in self-directed services
Provide instruction on navigating program rules and Financial Management Systems
Provide train-the-trainer instruction on identification and reporting of suspected fraud
Maintain documentation of services provided in accordance with policies and procedures
Perform all functions necessary to support enrollment including processing enrollment documentation
Communicate referral corrections to case management entities
Update enrollment status of participants and providers through portal and records in systems
Support workforce management delegation to ensure service levels are met
Document and report evidence of individuals' inability to self-direct appropriately

Required
Strong customer service and support experience
Proficient in Microsoft Office Suite, CRM, Five 9, My Account platforms and web-based applications
High aptitude for process assessment, improvement, and recommendations
Exceptional verbal and written communication skills
Ability to develop strong working relationships with external and internal stakeholders
Education: Related Bachelor’s Degree Preferred; can be substituted with 1+ years of related experience
Experience: Minimum of one (1) year of experience serving individuals with disabilities and/ or aging adults preferred
Other Requirements: Criminal Background Check
A valid driver’s license, access to a motor vehicle, and active vehicle registration, inspection and insurance certification required
