Responsibilities
Coordinate transitions of care for TRICARE beneficiaries through inbound and outbound calls
Utilize clinical and analytical skills to identify care coordination needs and facilitate care during transitions
Communicate with community providers, Military Treatment Facilities, and Veterans to coordinate care
Ensure authorizations are in place to meet clinical care needs and document coordination activities accurately
Participate in Quality Improvement Projects and report potential quality or fraudulent cases as needed

Required
Licensed Practical or Vocational Nurse (LPN/LVN) with a current unrestricted license in the appropriate state or territory
Minimum of 2 years of clinical nursing experience
Proficiency in medical data entry, written skills, and Microsoft Office
Exceptional oral communication skills and strong organizational abilities
Preferred
2+ years of Managed Care experience preferred
3 years of clinical experience
Experience working with TRICARE or military health systems
Familiarity with Quality Improvement Projects
