Responsibilities
Pre-register and schedule patients for appointments, procedures, tests, and other services
Handle incoming calls, complete follow-up and referral work queues, and manage ticket scheduling
Communicate with patient care teams to ensure efficient and appropriate care for patients
Accurately enter data into the electronic health record while interacting with patients
Support various population management and chronic disease management initiatives through patient registry and outbound calling

Required
Minimum of 1 year of clinical healthcare experience, or 1 year as a Patient Scheduler I, or 1 year in a contact/call center
Completion of designated HBI Courses within one year of hire
Ability to obtain required signatures and documents from patients
Proficiency in data entry and managing high call volumes
Strong customer service skills while working as part of a care team
Preferred
Experience in a healthcare setting with knowledge of patient scheduling processes
Familiarity with electronic health record systems
Strong communication and interpersonal skills
Ability to multitask in a fast-paced environment
Previous experience in population management or chronic disease management initiatives
