Responsibilities

Analyze an assessment that incorporates structural and process of care indicators to improve health literacy and communication between pharmacy and patients during the medication management encounter.
Conduct a longitudinal research and quality improvement project for presentation at a professional platform.
Develop a structural Certificate Program in the area of health equity, disparity and systems thinking for preceptors and pharmacist leaders.

Required

Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) with a minimum of a Bachelor’s Degree.
Licensure in good standing as a Registered Pharmacist; willingness to reciprocate.
Proficient computer skills using Pharmacy systems and Microsoft applications – Outlook, Word, PowerPoint, Excel.
Effective interpersonal and communication skills - verbal and written.
Ability to collaborate effectively with all levels across the organization.
Through training and/or experience, possess necessary knowledge to provide oversight of medication regimens for the patient populations treated in the facility including a knowledge of dosing for specific age populations.
Complete application submission to include curriculum vitae (CV), personal letter of intent and three letters of recommendation or reference.

Preferred

Doctor of Pharmacy degree.
Post-graduate pharmacy residency (PGY1) or equivalent training.
Public Health degree or public health services experience.
