Responsibilities
Provide overall project management and coordination.
Identify and engage partners with cultural and subject matter expertise to support implementing type 2 diabetes management and prevention activities in the U.S. territories and freely associated states.
Lead and conduct a needs assessment of DP25-0024 recipients’ needs to support the successful implementation of the diabetes strategies.
Create a dissemination plan with partners of the needs assessment.
Develop an evidence-based and culturally appropriate technical assistance plan based on the needs assessment of type 2 diabetes management and prevention activities.
Develop guidance documents to support DP25-0024 recipients in implementing strategies for diabetes.
Oversee the development of culturally and/or linguistically adapted program curricula and supplementary materials needed to implement DP25-0024 diabetes management and prevention activities.
Attend project team meetings with ASTHO, CDC DDT, and other identified partners.
Ensure collaboration within the Diabetes Portfolio, Center for Public Health Leadership (CPHL), and other internal and external partners.
Collaborate with other Portfolios and other Councils to find collaboration opportunities, share successes/ideas, etc.
Collaborate with the Expanding the Non-Communicable Disease Collaborative in the Islands project team.
Complete required monthly programmatic reports and monthly invoices.
Qualification

Required
Master’s degree in public health, public health administration, or related field.
Minimum of ten years of experience in the public health field.
Experience with diabetes self-management education and support and the National DPP lifestyle change program.
Experience in diabetes or other chronic disease states, risk factors, and/or health equity.
Experience convening diverse groups of partners and facilitating meetings with focused agendas for public health action.
Ability to work across time zones of many diverse partners.
Demonstrated ability to build relationships and strong project management skills.
Strong interpersonal and communication skills to build relationships with diverse partners.
Strong meeting facilitation skills.
Strong understanding of learning approaches.
