Responsibilities
Facilitate TCM by identifying patients who have been admitted to an inpatient, post-acute, or community setting.
Act as resource for TOC RN, PHP care manager, hospital case manager.
Track admission and work with hospital, TOC RN and skilled nursing facility staff to track admission and facilitate discharge planning.
Maintain tracking tools and update patient charts with updates on patient status.
Facilitate faster hospital discharge to skilled nursing facility by coordinating needs with members of care team.
Schedule follow up appointments and transportation as needed.
Coordinate patient needs to ensure they have the medication, care orders and durable medical equipment they need upon returning back to the skilled nursing facility.
Maintain knowledge of Medicare benefits, Plan benefit plans, extra benefits, case management, and utilization management procedures, community resources, principles of self-determination, and patient-centered care.

Required
Minimum of 3-5 years’ in health care related field, geriatric population preferred.
Ability and experience utilizing a variety of applications and databases to fulfill care management coordination requirements/documentation.
Must have integrity and a strong ethical compass.
Strong planning and organizational and time management skills with the ability to work independently.
Must have strong communication skills and the ability to work effectively with people coming from diverse cultural and professional perspectives.
Excellent interpersonal, written, and organizational skills required.
Must be a proactive and effective, creative problem-solver.
Preferred
Associate degree preferred.
Experience in Medicare Advantage programs preferred.
Experience in data analysis preferred.
Highly functional with Excel and data reporting.
Knowledge of current models or standards of transitional care management.
