Responsibilities

Conduct monthly summary reports on patients' health conditions based on the collected data, identifying medical changes or abnormalities for NP clinical review and treatment plan.

Complete medical chart audits and clinical documentation review.

Serve as a point of contact for questions and escalations.

Adhere to all established policy/protocol of the department and workflow.

Meets established standards for productivity as assigned.

Develops relationships within TapestryHealth to facilitate communication and improvement of program.

Works with Tapestry IT and Data Scientists to resolve discrepancies with patient data information.

Protects patient information and adheres to standard of confidentiality.

Contribute and participates in continuous quality improvement activities.

All other duties as assigned.

Required

Some college/technical education

Minimum of 3 years’ health field related work or volunteer experience

Proficiency in navigating electronic health records.

Proficient with Microsoft Office Suite.

Ability to understand and interpret basic math logic such as averages and graphs

Preferred

Associate degree or higher

Strong verbal/written communication

Ability to organize and prioritize tasks

Ability to work collaboratively, be attentive to detail and maintain a positive attitude.

Ability to educate internal and external Tapestry Health service line customers

Familiarity with HIPAA regulations and patient privacy standards
